-The best indicator of renal function than either that of urea or uric acid.

– A waste product of muscle contraction, which is filtered filtered by kidney and not reabsorbed but it’s excreted from, renal tubular cells,

– serum creatinine increase in muscular dystrophy & renal disease.

Creatine phosphate – phosphoric acid = creatinine.

Creatine – water = creatinine.

Principle:

Creatinine + alkaline picrateàorange- red color.

-80-90% of the red color is produced from creatinine and the rest will result from other substances such as ketones, glucose and ascorbic acid.

-By adsorbing creatinine on Lloyed’s reagent then read the absorbance for the interfering chromogens, the difference is equal to creatinine absorbance.

-More specific, non-Jaffé assays have also been developed. One of these, an enzymatic method, measures ammonia generated when creatinine is hydrolyzed by creatinine iminohydrolase. Its simplicity, precision, and speed highly recommend it for routine use in the clinical laboratory.

The normal serum creatinine:–

The normal serum creatinine ( sCr) varies with :

– The subject’s body muscle mass

– With the technique used to measure it.

normal range:–

For the adult male, the normal range is( 0.6 to 1.2 mg/dl), and( 0.8 to 1.5 mg/dl) by the older manual Jaffé reaction.

For the adult female, with her generally lower muscle mass, the normal range is( 0.5 to 1.1 mg/dl) by the kinetic or enzymatic method.

Serum creatinine increased in:–

Renal failure (both acute & chronic).
